Step One: Know Your Program Administrator
Your program administrator is the administrative contact for your residency/fellowship program and is here to assure that you have an optimal educational experience. He/She may have additional program-specific items for you to complete. Please bring your final diploma to orientation, as your program administrator will need to review it.

Step Two: Complete These Hospital Forms
To facilitate a seamless entry into your role as a resident/fellow at Scott & White and our major participating institution, please complete all hospital forms for Scott & White and Central Texas Veterans Healthcare System (CTVHCS).

Part Two
Central Texas Veteran Healthcare System (CTVHCS) Forms
All CTVHCS forms need to be completed, signed, dated and returned at least six weeks prior to the start of orientation. If a section does not apply, please enter N/A in that blank. Any questions regarding the CTVHCS forms should be directed to Teresa Greiner

NOTE: The CTVHCS forms only need to be completed if your specialty is: Anesthesiology, Cardiology, Dermatology, Endocrinology, Family Medicine, Gastroenterology, General Surgery, Infectious Disease, Internal Medicine, Medical Oncology, Nephrology, Ophthalmology, Orthopedic Surgery, Otolaryngology, Pathology, Plastic Surgery, Podiatry, Psychiatry, Pulmonary Disease/Critical Care Med, Radiology, Rheumatology, Urology and Vascular Surgery.

Step Three: Obtain Your Physician-in-Training Permit
You will need to apply for your Physician-in-Training permit online through Texas Medical Board (TMB):
NOTE: To participate in a Texas A & M-Scott & White graduate medical education program, a resident or fellow is required to hold either a Texas Medical Board (TMB) Physician-In-Training Permit, or a Texas Medical License. Also, TMB suggests that you print each screen, as well as the review screen, of your application as it is completed for your records. You will not be able to save your application. Please apply for your permit no later than April 15, 2012. Be sure to include an email address for TMB correspondence.

Step Four: Obtain Your National Provider Identifier Number (NPI)
NOTE: You will need to complete Step Three: Obtain your Physician-in-Training permit before you can apply for your NPI #.
You will need to apply for your National Provider Identifier online through The National Plan and Provider Enumeration System (NPPES).
* For Health Care Providers
The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) mandated the adoption of a standard unique identifier for health care providers. The National Plan and Provider Enumeration System (NPPES) collects identifying information on health care providers and assigns each a unique National Provider Identifier (NPI).
Step Five: Orientation and Training Schedule
Orientation
Orientation will be conducted between June 25 - June 29, 2012, beginning at 7 a.m.
Continental breakfast will be available at 6:45 a.m.
