Forms That Need to be Returned at Orientation (June 29)
You will have the opportunity to complete these forms in DocuSign electronically; however, you will also need to print them to bring to your Program Administrator at orientation. Please be aware that a local address is required on these forms.

The mission of the Scott & White Medical Education Alliance is to support the spouses of medical students, residents and fellows of Scott & White Hospital - Temple throughout their medical training here in Temple, Texas. Our members have the unique opportunity to meet other individuals during their stay at Scott & White and develop life-long friendships. The Alliance provides support to its members through adult socials, play groups, children's socials, philanthropic events and monthly meetings that are both informative and entertaining. The Alliance also provides support to local charities, including a monthly bake sale that benefits Scott & White.
